Caroline Wheeler is a highly experienced Registered Nurse and Independent Nurse Prescriber with a specialist focus on women’s health. She has undertaken the British Menopause Society Advanced Menopause Training and is a proud member of the British Menopause Society. In addition, Caroline is a Diplomate of the Faculty of Sexual and Reproductive Healthcare, with advanced training in coil and contraceptive implant fitting.

Alongside her private practice, Caroline continues to run NHS women’s health clinics, where she supports patients with menopause care and contraception services. She is also actively involved in presenting at educational events for both patients and healthcare professionals, sharing her expertise to raise awareness and improve standards of care.

Her approach is rooted in providing holistic, evidence-based care, ensuring that women feel heard, supported, and empowered throughout their menopause journey. A coil, also known as an intrauterine device (IUD) or intrauterine system (IUS), is a small T‑shaped device placed inside the uterus to provide long‑acting, reversible contraception. Some coils also offer additional health benefits, including the management of heavy menstrual bleeding and endometrial protection as part of hormone replacement therapy (HRT).

Hormonal Coil (IUS)
The hormonal coil releases a small amount of progestogen (levonorgestrel) directly into the uterus. It provides effective contraception for 3–8 years depending on the type.

Benefits may include lighter or absent periods, reduced menstrual pain, and protection of the womb lining when used alongside oestrogen HRT.

Copper Coil (IUD)
The copper coil is hormone‑free and prevents pregnancy through the action of copper. It lasts between 5–10 years and may be suitable for those who prefer a non‑hormonal option.

Your consultation includes a full medical and gynaecological history, discussion of symptoms and lifestyle, review of previous contraception, and an explanation of benefits, risks, and alternatives. There is time to ask questions and make an informed decision. In many cases, consultation and fitting can be completed during the same appointment.

The coil is gently inserted through the cervix into the uterus. Some cramping or discomfort may be experienced during or shortly after insertion. Pain relief options and aftercare advice are discussed in advance to support comfort.

Mild cramping or light bleeding is common in the days following insertion. Most people can return to normal activities the same day. Clear aftercare advice is provided, including when to seek medical review. A follow‑up check may be offered if required.

Your initial consultation begins with a detailed discussion of your:

  • Menstrual and reproductive history
  • Menopausal symptoms and how they affect your daily life

  • Current medications and allergies

Treatment Option 1 – Hormone Replacement Therapy (HRT)

If suitable for you, HRT can support you in multiple ways:

  • Replaces the oestrogen and/or progesterone your body no longer produces
  • Can significantly reduce hot flushes, night sweats, mood changes, and vaginal dryness
  • Offers protective benefits for bone and heart health in many women
  • Available in multiple forms – tablets, patches, gels, and sprays
  • Tailored to each individual after reviewing risks and benefits

Treatment Option 2 – (Strictly Non-Hormonal Option)

If HRT isn’t suitable or preferred, we can discuss alternatives such as:

  • Non-hormonal prescription medications that help ease certain symptoms
  • Vaginal moisturisers or lubricants for dryness and discomfort
  • Nutritional supplements (e.g., calcium, vitamin D, phytoestrogens – where appropriate)
  • Cognitive behavioural therapy (CBT) for managing mood or sleep issues
  • Holistic approaches: acupuncture, yoga, and mindfulness

Follow-Up and Ongoing Care

Your care doesn’t end after the first appointment.

  • Initial follow-up: usually 3 months after starting or adjusting treatment, to monitor progress and make changes if needed.
  • Regular reviews: typically every 6–12 months, or more often as advised.
  • Ongoing support for symptom management, lifestyle guidance, and prescription updates.
